If you’re willing, we’ll schedule a visit with you to get to know you better and for you to get to know us better, which will help you decide which care program might be beneficial to you.

Describe your care team and who will be caring for you and your loved ones

Ensure you understand the consent form you will be asked to sign if you are admitted onto our services

Ask for permission to speak with your doctor and other professionals as necessary about the care you require

The initial visit will be followed by another visit from a member of your care team.
Together you will develop a plan of care that will enable you to remain in control and live your life according to what is important to you.
