Enhanced Care Management (ECM) provides compassionate, personalized support for eligible Medi-Cal members with complex health and social needs. This no-cost program connects you with a dedicated Care Manager who coordinates your care, helps you navigate the healthcare system, and links you to community resources that support your overall health and well-being. Whether you need help accessing medical care, housing, transpo1tation, food assistance, behavioral health services, or other community supports, we're here to help you reach your health goals with care tailored to your unique needs.

How ECM Helps:

• Community-Based Care - Care comes to you. Your dedicated Care Manager meets with you in your home or community, making it easier to get the support you need.

• A Personalized Care Plan - Together, you and your Care Manager build a care plan based on your goals, values, and individual needs.

• Coordinated Support- We connect you with medical care, behavioral health services, and vital community resources like housing, food, and transportation.
