Hospice
When is it time to consider Hospice?
If you have questions about hospice care, you're not alone. Understanding your options can bring clarity and peace of mind during a difficult time.
Hospice care is designed for individuals who have been diagnosed with a terminal illness and whose physician believes they have a life expectancy of six months or less, should the illness follow its expected course. The focus of hospice isn't on curing the illness - it's on providing comfort, managing symptoms, and enhancing quality of life, while supporting both the patient and their loved ones through the end-of-life journey.
Lake Circle of Care is here to provide information, answer your questions, and help you determine whether hospice care may be the right choice for you or someone you love.
For more information about our services, please call 707-263-6222.
The sooner you have the information, the sooner you can make informed decisions with greater peace of mind.
Your Hospice Care Team
Our Hospice Care Team is made up of a full complement of professionals who attend to the physical, emotional, and spiritual wellbeing of both the patient and their family. Together, the team works with the patient and family to build an individualized plan of care.
For most patients and caregivers, one of the greatest benefits is that our team comes to you - no trips to a doctor's office or clinic required. Visit frequency is based on each patient's needs, as outlined in the care plan, and adjusts as medical situations arise.

Spiritual Care
We care for body, mind, and spirit- that intangible spark that makes you, you, and the way you draw meaning from your life and relationships. For some, that involves faith, religious practice, or cultural tradition. For others, it doesn't.
Whatever your belief system or cultural background, our Chaplain is here to support you - whether that means caring for your spiritual needs directly or helping connect you with a faith leader who represents your beliefs. At Lake Circle of Care, we understand that those entrusted to our care come from diverse cultural, spiritual, and social backgrounds.
Our Chaplain helps patients and families explore spiritual distress or unmet spiritual needs, and remains available for counsel and consultation. Trained to meet each person where they are, the Chaplain's role is to listen, comfo1t, and guide - offering insight and inspiration when needed, and creating a safe space for reflection and self-realization, because every end-of-life journey is unique.
Above all, the goal of Spiritual Care at Lake Circle of Care is to help patients and families find their own comfort and peace, and to provide the support they need to make the most of the time they have together.
"Concerns about.faith, meaning and.forgiveness are common to us all. We are available to act as a listening ear and a peaceful presence during this time. "
~ Chaplain Rick Barnes
