Palliative Care

What is Palliative Care?

Support for the Whole You

Living with a serious illness can be overwhelming, but you are more than your diagnosis. Our Palliative Care program offers an extra layer of support to help you manage the physical and emotional challenges that come with a life-limiting or chronic illness.

What We Offer

Our caring, professional team provides home-based care focused on relieving symptoms like pain, fatigue, and stress. We work in partnership with your healthcare providers to ensure you receive personalized, coordinated care that meets your unique needs.

Benefits of Palliative Care:

•    Improved comfort and symptom relief
•    Enhanced quality of life
•    Support for emotional and spiritual well-being
•    Reduced emergency room visits and hospital stays

Whether you're navigating treatment or managing a long-term illness, we're here to help you focus on what matters most.

Who Qualifies?

Palliative Care is available to individuals ages 18 and older with a qualifying serious illness. There is no cost to qualifying patients whose primary coverage is Partnership HealthPlan of California.
